Return to TopPatients can ask to sign up during an office visit or self sign up by clicking the Sign Up Now button located on the www.uhmychart.org login page and completing the online form. After submitting the online form, the information is automatically checked to match you with your medical record. If a record is found matching your information, you can create a user name and password to complete the activation. If no record matching your information is found, our Health Information Management team will contact you.

Return to TopYes you can. This is called Proxy access and there are two types of access: Adult to Adult and Adult to Child. Adult to adult proxy allows you to access an adult's (over 18) medical information via MyChart. Adult to child proxy allows a parent or guardian to access a minor's medical information via MyChart. Proxy forms are available for download on the www.uhmychart.org log in page under the "Requesting access to a family member's MyChart" heading. Instructions for submitting the request are included at the bottom of each form.
Return to TopNo. A University Health Care System medical record is not needed. A non-patient proxy is a user with a MyChart account, but no patient record.
Return to TopA proxy can send a message to the office of the patient's primary care physician or any provider with whom the patient has had an appointment in the past year at a participating University Health Care System facility or subscribing provider. If you are not a proxy for your family member, please do not send messages about that person from your MyChart account, since the information would not appear in the correct person's electronic medical record.

Return to TopTo retrieve your username click on the "Forgot MyChart username" link on the log in page and answer the personal information questions to have your username sent to your email address. To reset your password, you may click on the "Forgot password" link. After answering a security verification question you will be able to create a new password. If you require additional assistance, contact our 24-hour MyChart Support Line at 706-774-5800.
Return to TopLog into MyChart and go to the Preferences tab. On the drop down menu select Demographics to update your address and contact information. You can change your password and security question under Security Settings. Email address settings are available under Notification Settings.

Once you have created a user name and password at www.uhmychart.org, you can download the free MyChart app from the iPhone App Store or Android Market by searching for MyChart (Epic). After installation, the app will ask you to select a state, select Georgia and then select University Health Care System from the app's phone book. Note that not all MyChart features are available from the mobile app.

Return to TopYou will receive an e-mail when you have new information in your MyChart account. If you are not receiving these e-mails, please verify that your current e-mail address is set in your preferences and that you have elected up to receive notifications. To do this, log into MyChart and on the Preferences tab, click "Notification" on the drop down menu. On the notifications page you can select which type of alerts you would like to receive notifications as well as change your contact e-mail address. You may also want to check your personal e-mail settings to make sure MyChart e-mails are not being sent to your junk mail folder.

Your MyChart information comes directly from your electronic medical record at the hospital and your provider's office. You can submit update requests from the Current Health Issues, Medications or Allergies sections under the My Medical Record tab. Changes to your health information will be reviewed and updated at your next visit.
Return to TopYou can submit a request to schedule an appointment with a participating physician that you have seen in the past year at University Health Care System facilities. Additionally, many University Health Care System locations offer online scheduling directly through MyChart. With direct scheduling you can choose from a list of available dates and times in which to schedule an appointment.
Return to TopYou can cancel your appointment up to 24 hours prior to the appointment. If you are within 24 hours of your appointment and wish to cancel, please call the practice or department.
Return to TopYou will generally receive an answer within 3 business days; however, staff and information availability may result in a longer response time. Please note that MyChart should not be used for urgent situations. Please contact your provider's office if the situation requires immediate attention or dial 911 if it is an emergency.
Return to TopWe take great care to ensure your health information is kept private and secure. Access to information is controlled through secure personal IDs and passwords. Each person controls their password, and the account cannot be accessed without that password. Unlike conventional e-mail, all MyChart messaging is done while you are securely logged on to our website.
Return to TopMyChart is fully compliant with federal and state laws pertaining to your privacy. Your name and e-mail address will be treated with the same care and privacy given your health records and will never be sold or leased by MyChart.
Return to TopFor security purposes, if your keyboard remains idle for more than 15 minutes, you will be automatically logged out of MyChart.
